The issuance of athlete health certificates (KOA / CSO) requires that you have with you at the appointment:
1. Medical examination form (KOA / CSO website)
2. Required tests (KOA / CSO website)

Once the required tests have been performed, please contact us for an appointment.

Note: The required tests (different depending on age) are not covered by the GESY and you must contact the sports federations for collaborating Hospitals where they can be performed.
